social: the personal relationship and trust between patient and (the team of) healthcare professionals
clinical: the actual care
structural: the buildings, facilities and systems used in providing care
The patient needs outlined above manifest themselves most strongly in the social and clinical dimensions: the patient wants to be well-informed, understand what is wrong with him/her and what awaits him/her, feel understood and be treated well and as quickly as possible. Structures (the third dimension) are prerequisites for the patient: they must be there and they must work. They are dissatisfiers; if they are not in order, it leads to dissatisfaction.
Every patient goes through their own care process with a number of steps in which their needs manifest themselves. The Patient Cycle Hospitals, drawn up by Present Media, essentially shows how such an individual care process proceeds. This cycle offers a practical tool to outline the needs of patients in each phase.

By plotting these need categories on the patient life cycle, a good picture of the patient focus per phase is created (.docx). To illustrate the whole, I have included a number of examples in this document.
